Instructions for Completing Online Forms
- Please complete one Loan Application form for the business.
- While only one business owner is required to electronically sign the completed Loan Application, please ensure to list ALL owners on the form.
- The Personal Statement of Affairs (PSA) should only be submitted AFTER the Loan Application has been completed and submitted.
- Each business owner / shareholder must complete a separate Personal Statement of Affairs. Please select “Start a New Form” to re-set for additional PSA submissions on the same device.

Be prepared to also upload the following documents:

Any legal documentation that relates to the business, if applicable, such as:
- Business Name Registration
- Articles of Incorporation
- Partnership or Shareholder Agreement(s)
- Any other Agreements such as Leases, Customer Contracts, etc.

Be prepared to upload the following documents on the PSA:

Photo identification, such as a Driver’s License or Passport.

- These forms can only be completed and submitted electronically. Please follow the step-by-step instructions and prompts.
- These forms will store your information for three months/90 days before, and after, submitting.
- Once submitted, make sure to download a copy of the completed documents for your records.
- Additional documents may be requested after review of your submission.
